Birth Certificates

A Birth Card or a Birth Certificate can be obtained right here in the Clerk’s Office. You can receive a Birth Card or Birth Certificate if you were born in Louisiana from 1915 to present. However, your birth record must be on file at the Vital Records Office. Requirements for obtaining a Birth Card or Birth Certificate: 

• Pictured ID
• Applicants name.
• Date of Birth
• Parish or city in which applicant was born.
• Parent’s Names.
• Mother’s Maiden Name.

Document Fees

A Birth Card can be purchased only with purchase of a Birth Certificate.
The fee for both is $48.00

Death Certificates

Death Certificates from the period of 2011 to present can also be obtained in the Clerk’s Office. Only immediate surviving family or beneficiaries can obtain a death certificate. Beneficiaries must have a copy of the insurance policy showing the applicant’s name listed as a beneficiary or a copy of the last will and testament naming the applicant as beneficiary.

Requirements for obtaining a Death Certificate:

• Pictured ID
• Applicants name.
• Date of Death
• Parish or city in which applicant died.
• Parent’s Names.
• Mother’s Maiden Name.
